Our Goal
The goal of your Carolina Community Health Partnership Nurse Care Manager is to help you utilize the medical system in a way that will ensure that you receive quality, cost-effective medical care as well as other needed services. Your nurse care manager can help eliminate barriers that may get in the way of your being able to access the medical care and services you may need by working with primary care providers and other community agencies.
What can a Nurse Care Manager can do?
Your Nurse Care Manager can:

  • coordinate services requested by the primary care doctor for you
  • access formal and informal resources for you
  • link you to needed services and agencies
  • monitor you progress
  • help educate you regarding your disease
  • help you understand your medications
  • help educate you regarding the proper use of medical services including the emergency department
  • help answer your questions about your medical care

Available Services
To ensure quality of care, your Carolina Access Primary Care Provider offices offer the following services:

  • Asthma Disease Management
  • Diabetes Disease Management
  • Emergency Room use management
  • Management of chronic health problems
  • Prescription medication coordination.

Nurse Care Managers are Registered Nurses who can help answer questions about your healthcare.
