Case Management Services
Our Goal/Mission
As RN Case Managers, we will function as a liaison and resource to Primary Care Providers (PCPs) to promote optimal patient outcomes. We will strive to improve quality of care by promoting use of the medical home.
What Case Managers do?
- Support and educate your patients on your plan of care for them.
- Provide patient education related to:
- Disease management
- Appropriate use of medical home
- No show appointments
- Pharmacy utilization
- Efficient use of Medicaid benefits:
- post appointment visit follow up
- choice of medical home
- appropriate use of ED
- 24 appointment visit limit
- transportation choices
- specialist referrals
- after hours PCP availability
- Focus on disease prevention/health promotion
- Referral coordination
- accept PCP patient referrals
- accept hospital referrals
- accept community service provider referrals
- facilitate miscellaneous referral requests.
- Investigate and share information pertinent to PCP plan of care
- Act as liaison for:
- patients and their families
- PCPs and patients
- PCPs and other providers
- patients and community service providers.
Case Managers will also assist with implementation of Community Care of North Carolina Disease Management Initiatives in offices by:
- helping with the development of group visits
- educating office staff on disease management initiatives
- help with disease management office process development related to individual office needs
- assist with interpretation of quality improvement audit results
- provide resource materials
- provide DME equipment
- provide telephonic case management as needed
- educate offices on how to refer to case management.
